This form is needed to collect your personal information, and also your signature acknowleding that you understand the rules of playing in our league and accept our bylaws.

NCASA Registration Form

NCASA is our governing body. They administer amateur soccer in our state according to USASA rules. This form gets sent to them, and registers you for liability insurance when on the field. No signature, no insurance! You also receive a players pass. Without this form it can not be issued.

Please note that our league year is according to the FIFA year. From Fall to Spring. Every league year all players must register with NCASA.

If you are already registered with NCASA for a different league, and want to play in the C.H.A.C. Soccer League, you will have to fill out a transfer form.
